Adults with a confirmed diagnosis of MS between the ages of 18 and 65 years were eligible to participate in this study. This age range was chosen because MS is most commonly diagnosed between ages 20 and 40 years, and the greatest number of Americans in current and projected population predictions lies within this range. Cognitive dysfunction occurs in approximately 50% of people with MS, and 40% report depressive symptoms. These conditions vary in severity; thus, patients with these symptoms were considered for participation. Cognitive and psychological incompetence, however, were exclusion criteria. No assessment of cognitive function was performed. For the purposes of this study, incompetence was defined as cognitive or psychiatric impairment making it impossible for the person to understand the consent form or survey. Examples of this include the need for a power of attorney to make medical decisions, and the need for assistance with activities of daily living due to cognitive, but not physical, disability.

Exosomal miRNAs are dysregulated in MS patients and differentially expressed between disease subtypes. Twenty-five unrelated individuals with a diagnosis of MS (relapsing-remitting n = 14, progressive MS n = 11 (SPMS n = 7, PPMS n = 4)) and 11 healthy individuals were studied. A second, independent set of progressive cases (n = 11) was then analysed; participant demographic and clinical characteristics are outlined in Table 1. The healthy control cohort was selected to match for age and gender to the RRMS group. While progressive MS is associated with older age and different gender ratio compared to RRMS11, Pearson correlation demonstrates that age, gender and treatment did not correlate with the expression profiles of the identified miRNAs (Supplementary Figure 2a). Also, these clinical characteristics when incorporated to the multivariate modelling have minimal contributions to the model’s prediction accuracy (Supplementary Figure 2b).

miRNAهای اگزوزومی در بیماران MS منظم نیستند و بین زیر دسته های بیماری به شکل متفاوت نمود می کنند. 25 فرد بدون نسبت فامیلی مبتلا به MS (14 تا پیشرونده - عودکننده و 11 پیشرونده (7 تا ثانویه و 4 تا اولیه)) و 11 فرد سالم مورد بررسی قرار گرفتند. بعد از آن گروه مستقل دیگری از مبتلایان به MS پیشرونده (11 فرد) تجزیه و تحلیل شدند، اطلاعات آماری و بالینی شرکت کننده ها در جدول 1 آمده است. گروه کنترل افراد سالم برای همسان بودن از نظر سن و جنس با گروه پیشرونده-عودکننده انتخاب شدند. در حالی که MS پیشرونده با سن بیشتر و جنسیت متفاوتی نسبت به پیشرونده-عودکننده پیوستگی دارد. همبستگی پیرسون (Pearson correlation) نشان داد که جنسیت، سن و درمان ارتباطی با نمایه بروز miRNAهای شناسایی شده وجود ندارد (شکل تکمیلی 2a). همچنین وقتی که این مشخصات بالینی در مدلسازی چند متغیره گنجانده شدند سهم کوچکی در دقت پیش بینی مدل داشتند (شکل تکمیلی 2b).

Parental orders and the rights of surrogate mothers
Obstetricians need to be aware of potential legal changes which may impact the doctor–patient relation in surrogacy cases. At present, English law recognises the surrogate as the mother. After giving birth, she may hand over the baby.

متخصصین زنان و زایمان باید از تغییرات حقوقی که ممکن است بر رابطه پزشک و بیمار در موارد رحم اجاره ای تاثیر بگذارند آگاه باشند. در حال حاضر طبق قانون انگلیس مادر جایگزین مادر محسوب می شود. بعد از بدنیا آوردن ممکن است بچه را واگذار کند. سپس زوج مقابل درخواست حضانت می دهند و اگر شرایط قانون رحم اجاره ای تنظیم شده در سال 1985 را داشتند والدین قانونی نوزاد خواهند شد.
